Practitioners in Kings, Bronx and New York, New York Counties for
in-home health assessments on Medicare and Medicaid Advantage
Members.Key Details: Competitive pay per assessment Flexible
schedules, open 7 days a week Minimum 10 hours per week commitment
Paid training and provided equipment W2 position with weekly
payResponsibilities: Conduct Adult/Geriatric Assessments Manage
medical history, diagnosis, treatment Provide health education,
referrals, and case management Collaborate with medical
personnelQualifications: 6+ months of NP experience Master's degree
Board certified by AANP, ANCC Specializations in either Family
Health or Adult Gerontology Current BLS certification
