Job Description FlexStaff is seeking Social Workers with various
levels of experience to work for one of our clients, the largest
city health system for their location in Queens, NY. This is a
permanent, full-time job offering benefits. Social Worker Level 2:
Requirements : LMSW or LCSW Schedule: 8:00AM-4:00PM Salary: $85,078
Social Worker Level 3: Requirements : LCSW Schedule: 8:00AM-4:00PM
or 9:00AM-5:00PM Salary: $95,713 Social Worker Level 4:
Requirements : LCSW and 1 year of supervisory experience Schedule:
9:00AM-5:00PM Salary: $106,348 In this role you would be
responsible for providing comprehensive psychosocial support to
patients and their families within the hospital setting. They
assess patient needs, facilitate discharge planning, connect
individuals to vital community resources, offer emotional support,
and advocate for patient well-being, ensuring continuity of care
and mitigating barriers to recovery. Responsibilities include, but
not limited to: Conduct rapid psychosocial assessments for patients
and families facing acute medical conditions, trauma, or
end-of-life. Provide crisis intervention, brief counseling, and
emotional support to help patients and families cope with illness,
injury, and complex medical decisions. Lead and coordinate
comprehensive discharge planning, ensuring safe and timely
transitions to post-acute care, home, or other appropriate
settings. Identify and connect patients and families to vital
community resources, including home health, long-term care, mental
health services, financial aid, and transportation. Act as a core
member of the interdisciplinary healthcare team, collaborating with
physicians, nurses, and other specialists to optimize patient care.
Advocate for patient rights, ethical considerations, and addressing
social determinants of health impacting recovery and discharge.
Maintain accurate and timely documentation within the electronic
health record (EHR), adhering to hospital policies and regulatory
standards.
